Meet Savannah Platt
We are pleased to introduce our newest provider, Savannah Platt, MSN, FNP-C, in our mission to provide exceptional hormone management for the Fort Wayne community. Savannah is certified through the Hormonal Health Institute and collaborates with Dr. Bauer to treat both men and women. Call to make your appointment today! Savannah is a board-certified nurse practitioner passionate about helping patients feel their best through personalized, one-on-one care. She earned both her Bachelor of Science in Nursing and Master of Science in Nursing from the University of Saint Francis. Savannah began her nursing career in the Cardiac Intensive Care Unit, where she developed a strong foundation in critical care, and later expanded her expertise as a nurse practitioner in dermatology and aesthetics.
At Bloom Hormone Health, Savannah will specialize in hormone replacement therapy, thyroid management, and weight management, creating customized treatment plans tailored to each patient’s needs and goals. Savannah is dedicated to building trusting relationships and empowering patients through education and ongoing support.

Outside of work, Savannah enjoys spending time with her husband, Konner, and their son, Knox, as well as staying active at Orangetheory Fitness.
